  • There is a non-refundable $199 Initial Office visit with Dr. Pervaiz, MD. This visit will include a thorough discussion regarding your concerns and any questions you may have as well as a treatment plan and recommendations from the physician.

  • There is a $50 deposit required to book and confirm your appointment.

    If you do not cancel within 48 hours or don’t show up for your appointment, there will be a $50 cancellation or no-show fee.
